An easy way to avoid having any problems when you are attempting to open files is by checking what directory you are in, and where you are making the call to. Create a simple PHP page and make a call print the current directory. The problem was that fopen() was failing when trying to access a file as a URL through apache -- even though it worked fine when run from the shell and even though the file was readily readable from any browser. I am not very familiar with CentOS and its specific permissions issues with httpd+php environment so that I got stuck with "failed to open stream: Permission denied" after installing website on production server.
